This clinic is only taking United and UMR insurance at this time.  If you want to use your United or UMR benefits, please verify the number of treatments you are allowed per year, your co-pay, and if your treatments are subject to your deductible before your visit. 

If you have another insurance carrier, we can quickly provide a super-bill with ICD-9 codes for you to send to your insurance company for reimbursement. If you require reimbursement, checking with your insurance company before getting treatment is best.
